arrange to take your picture at the hospital.
STAFF: include your favorite things survey or
indicate that you need one emailed to you.
PATIENTS: include your name, age at diagnosis, and
anything you want others to know about your story in about 100 words or less. If you want a
favorite things survey, let us know and we will email you one.
Within one week, you will receive a proof to review via email and a consent form.
